To become a patient at Christ's Family Clinic you MUST provide the following:
- A completed New Patient Registration Packet (Can be picked up during administrative office hours)
To make an appointment for your first visit to Christ’s Family Clinic, bring the required documentation, a Project Access Dallas consents packet and a completed new patient registration packet to the clinic during the hours listed below.

Para hacer una cita por un paciente nueva, traiga la documentación requerida a la clínica durante horas abajo. Usted también completará un paquete de registro para los nuevos pacientes y un paquete de registro de Project Access Dallas. Cuando usted ha presentado toda la documentación requerida le darán una cita.
